After completion of our online consent form, you will receive a Client ID # to proceed.  If there are contraindications in your responses to the “Yes” or “No” questions, a staff member will reach out for further discussion prior to issuing a Client ID #.

Your Client ID # is used to protect your personal information when providing online data.

Completion of the online assessment and scheduled follow up appointment to review (billed under Naturopathy) are non-refundable.
