Complete Consent Form

We need your consent to collect personal information and discuss eligibility for treatment.

Receive WMD Client ID

Once we receive your patient consent form, WMD will contact you by telephone.

Complete Online Assessment

With your registered WMD Client ID and online payment, you'll be provided instructions for starting the online health assessments.

Online Assessment Review

A practitioner will review your history and you will be contacted for a discussion to review and determine testing method and components.

Yes, we are welcoming new clients on a continual basis.

Unfortunately, we are unable to accommodate spontaneous onsite visits due to our commitment to previously scheduled appointments. Please follow the steps to Getting Started.
Our emails are triaged in order of urgency and our response may not be immediate either by phone or email.  If your concern is immediate or you have an acute health concern please contact your family doctor, visit an after-hours clinic or reach out to a telemedicine platform.  These avenues are equipped to address time sensitive symptoms.  If your concern is prescription related, the pharmacy that fills your prescription can be a great resource.
